Single text line with 'date' input type. You can use other standard and HTML5 input types: text (default), email, password, date, datetime, number, range.

Rating Table

Rating Table question allows to display table of multiple sub-questions with single option selection. Alternative 'Other' option can be used like in this variant. Both options and sub-questions can be either configured in question settings or auto-loaded via SQL.
