No, phpBB should be installed on same domain in 1st-level folder, i.e. in
http://www.site.com/phpBB3

If forum allows answering to topic for logged users only - user should be already logged in phpBB. Otherwise user will see 'Please login first' error message.

We don't have refunds without serious reason, here is our refund policy:

Hi,
- About the first issue - is there no way around it?
cannot the domain just be pointed at a different place ? (I know that jfusion does it for the bridge).
- So if I allow guest posts, they willl be able to post, but in order to post under the username, they have be logged on - got it.
- The refund policy is disappointing, other extension offer 30-day full refund (i.e. sh404SEF and more).
This way, I have no idea whether it will work properly on my site and test it. espcially if there is no easy solution to the first issue (forum in different sub-domain)

No, this is essential requirement. Actually it should work if using another domain for phpBB, but phpBB files should be located in 1st-level subfolder of Joomla domain, i.e.
http://joomla.com/phpBB3 if phpBB is loaded via subdomain
http://phpBB3.joomla.com/

Yes, user should be logged in phpBB in this case.

It's simple: please check all extension requirements, if they are met - it will work, otherwise it will not work. If requirements are met but it doesn't work - we will try to fix it, if we fail to fix it - we issue a refund.
